This document, officially known as a certificate from the Krajowy Rejestr Karny (KRK) , proves whether you have a criminal record in the Polish justice system.
For foreigners who no longer reside in Poland, obtaining this document can seem impossible. Polish consulates do not issue this certificate, and the online government portal (ePUAP/e-KRK) often requires a Trusted Profile (Profil Zaufany) or a Polish digital signature—something most non-residents do not have.

- The form is only in Polish.
